Orders received before 1:00 pm EST and currently in stock will ship same day.

If you need to pay by credit card, please call Accounting at 800.866.7257 ext. 203.

• In the “Name” field, type YOUR name (the person entering the order).
• In the “Facility/Doctor Name” field, type the Doctor’s name OR the name of the facility.
• In the “Phone Number” field, type in the phone number that is associated with the account for which you would like to order.
• In the “Customer Number” field, type the customer number linked to the phone number you provided in step 5. If you do not have this information, please contact Customer Service at 800 886 7257. We will be happy to provide this information.
• In the “E-mail Address” field, type in a valid email address. You will receive an email confirmation with a ship date and ETA. Please double check spelling when entering your email address to ensure our ability to respond to your order via email with confirmation.
• If you need to reference a purchase order (PO) for your order, please enter this information in the “PO Number” field. If you do not need to reference a PO, please leave this field blank.
• In the “Part Number” field(s), please enter the pack number/stock number you wish to order. This information can be found on a previous pack/stock box in large bold numbers/letters (ex: 40561BIP or 682-026).
• In the “Quantity” field, enter the appropriate quantity.
• Please make sure to list the correct unit of measure. For example, if your product comes 5 per case, you may enter 5 in the “Quantity” field and leave the “Unit of Measure” field at the default of “EA” OR you may enter 1 in the “Quantity” AND change the “Unit of Measure” field to “CS.”
• Repeat steps 9-11 for each item you wish to order.
• If your office has multiple shipping locations, please indicate to which location you’d like your order shipped in the “Shipping Instructions” field.
• If you have any other additional instructions for your order, please indicate this in the “Additional Comments” field.
• Click “Submit Order.”
• You will receive an email confirmation of your order within 1 business day.
